Why ESA invoices get rejected, and how to fix them
When a program rejects an invoice, providers using ChoicePaid record why. We count those reasons across every workspace and publish them here, so every provider can avoid the same mistakes.
What the data shows
Still collecting
We publish a reason for a program only once at least 5 rejections from at least 3 different providers have recorded it, so no provider can be identified. None have been recorded yet. This page updates daily.
Common reasons and the fix
Our guidance, not program rules. For each program's actual invoice requirements, with official sources, use the invoice builder or the program's guide.
- Student name missing or doesn't match
- Use the student's legal name exactly as the program has it (for example as it appears in the family's program account), then resubmit.
- Parent name missing or doesn't match
- Add the parent or guardian of record exactly as listed with the program.
- Missing or unclear dates of service
- List each date of service. Bill one line per session instead of combining them.
- Description too vague
- Name the subject or therapy and the length on every line, for example "Algebra 1 tutoring, 60 min, one-on-one".
- Credential or license missing
- Add your license or certificate number in Business details, keep a current copy in Documents, and attach it if the program asks.
- Amount or total doesn't match
- Check each line's quantity times rate, and that the total matches what the family requested in the payment platform.
- Duplicate of an earlier request
- Check whether the earlier invoice was already paid before resubmitting. Void this one if it was.
- Service or item not eligible
- Remove the ineligible item: in some programs one ineligible line rejects the whole request. Check the program guide's eligible expenses.
- Provider details missing or don't match enrollment
- Make your business name and address match exactly what you enrolled with in the program.
How this is collected
When a provider marks an invoice rejected in ChoicePaid, they pick the reason. We count only the program and the reason; no names, amounts, invoice contents or provider identities are used or published. A program and reason appear only above the thresholds above.
