Therapist requirements by state
Yes, in every program here, though Arizona limits therapy to students with disabilities. What changes is the license proof, the business paperwork and, in Texas, an insurance rule.

| ArizonaArizona ESA | Allowed only for qualified students with disabilities. Families upload a legible, unexpired copy of your license or accreditation (no screenshots), and your license number must be on the invoice.3 |
| TexasTEFA | An active professional license in your field. TEFA pays only for therapy not covered by Medicaid, CHIP or the child's private insurance. Fingerprinting for everyone who works with students.4 |
| GeorgiaPromise Scholarship | Licensed under Title 43, Chapter 10A, 28, 33, 34 or 44 of the Official Code of Georgia (occupational, behavioral, physical, speech-language and similar therapies).5 |

| LouisianaGATOR | Apply as a Therapist / Therapy Provider (OT, PT, speech-language, behavioral health, social work, vision, audiology and more) and upload a copy of your LLC.7 |
| UtahUtah Fits All | Business license with incorporation or IRS documents, plus professional license numbers and documents.8 |
| WyomingWyoming ESA | FEIN, business documents and copies of current licenses or certifications for the services you provide.9 |
| IowaStudents First ESA | Educational therapies qualify from vendors credentialed by the Iowa Board of Educational Examiners or another state-approved entity.10 |
